Description:	All proceeds of this Golf Scramble will go toward the support of the Taylor Regional Hospital Van Service. This FREE transportation service assists patients receiving services at the main hospital, physician offices, cancer center or rehab services who otherwise would not be able to provide their own transportation. We serve patients from Taylor, Green, Adair and Marion counties. $75 per person/$300 per team Sponsorship Opportunities are available: Major - $5000 Includes: registration fee, customized TRH golf shirt, tee sign, and one dozen Titleist ProV golf balls Platinum - $2500 Includes: registration fee, tee sign, and one dozen Titleist ProV golf balls Diamond - $1000 Includes: registration fee and tee sign Hole Sponsorship Levels: Bronze - $100 Silver - $250 Gold - $500
