Care Manager II (RN/LPN)

Job Description: Provides ongoing support & expertise through assessment, planning, implementation & overall evaluation of individual patient needs as they relate to continuing care post hospitalization. Accountable for communication with third party payors regarding patient's clinical status during hospitalization & post hospitalization when necessary. Will be responsible for preauthorization of medical needs within the hospital setting.

Education: Valid KY RN or KY LPN License.

Experience: 1-2 years' experience preferred.

PRN position available.
